Soccer Central participation form

I am over the age of 18 years.

I agree to have pictures taken and publicized in all forms (including print, web and social media) as part of the promotional and advertising efforts related to (Insert event here) and any other City publications.

I agree that my photos may be used for future promotional material across all Quinte West publications.

I will remain on-site for the duration of the event (Friday, Oct. 11, 2024 - Sunday. Oct. 13, 2024, from 10 a.m. - 5 p.m. daily).

I agree to provide valid insurance, naming The Corporation of the City of Quinte West as additionally insured, the name and date of the event, the address of the event, and 30 days written cancellation notice prior to the event.

The City of Quinte West is not responsible for any display materials, damage, loss, vandalism or other impacts to my personal property resulting from my participation in Soccer Central at the 2024 Canada Soccer Toyota National Championships U-15 Cup.
